What Is TRT?
Testosterone Replacement Therapy (TRT) is a medical treatment that restores testosterone to healthy levels in men experiencing hypogonadism or age-related decline. Testosterone isn't just about muscle — it's a critical hormone that affects energy, mood, cognitive function, bone density, cardiovascular health, and overall vitality.
Male testosterone levels decline approximately 1-2% per year after age 30. By age 50, many men have levels significantly below their youthful peak, leading to symptoms that are often dismissed as "just getting older."
How It Works
TRT supplements the body's natural testosterone through various delivery methods — injectable testosterone (cypionate or enanthate), transdermal gels, or pellets. The goal is to restore testosterone to optimal (not supra-physiologic) levels, mimicking the body's natural production.
When testosterone enters cells, it binds to androgen receptors, influencing gene expression related to protein synthesis, muscle growth, bone density, red blood cell production, and neurotransmitter balance. Some testosterone is also converted to DHT (for libido and prostate function) and estradiol (for bone health and brain function).
Key Benefits
- Increased muscle mass & strength — Supports lean body composition and physical performance
- Improved mood & mental health — Reduces depression and anxiety symptoms associated with low T
- Enhanced libido & sexual function — Restores sex drive and erectile function
- Better cognitive performance — Improves memory, focus, and verbal fluency
- Stronger bones — Increases bone mineral density, reducing fracture risk
- Cardiovascular support — Optimal testosterone levels are associated with improved lipid profiles and cardiovascular health
- Increased energy — Combats fatigue and improves motivation
- Better sleep quality — Supports restorative sleep architecture
What the Research Says
A landmark study in the New England Journal of Medicine (the TRAVERSE trial) involving over 5,000 men with pre-existing cardiovascular disease found that TRT did not increase cardiovascular risk — addressing long-standing concerns. The study also confirmed improvements in sexual function, mood, and walking distance.
Research published in JAMA Internal Medicine showed that restoring testosterone to normal levels in deficient men was associated with reduced all-cause mortality. The key is proper monitoring and dosing to maintain physiologic — not supra-physiologic — levels.
The American Urology Association recognizes TRT as an appropriate treatment for men with clinically low testosterone levels and associated symptoms, when properly monitored by a physician.
Important Considerations
TRT requires medical supervision. Regular blood work monitoring includes:
- Total and free testosterone levels
- Estradiol (estrogen) levels
- Hematocrit (red blood cell count)
- PSA (prostate-specific antigen)
- Lipid panel
When properly prescribed and monitored, TRT is a safe and effective intervention that can significantly improve quality of life for men with clinically low testosterone.
